Acute left MCA infarct hyperdense MCA sign

Noncontrast CT of the brain (left) demonstrates decreased attenuation with effacement of the cortical medullary junction along the left temporal lobe (black arrows), consistent with an acute infarct. The M2 and M3 segments of the left MCA are hyperdense, consistent with thrombus (white arrows, top right = magnification). Intracranial MRA demonstrates complete cut off of the left M1 segment (bottom right).
