Diagnosis: Pulmonary embolism - Intermediate probability on V/Q scan

Pulmonary embolism
Frontal chest radiograph demonstrates an opacity projecting over the medial right lung base. Patient presented with acute chest pain and a V/Q scan was ordered to rule out pulmonary embolism (see second image).

Intermediate probability for PE on V/Q scan
V/Q scan (second image) demonstrates multiple nonsegmental matched ventilation/perfusion abnormalities. A matched ventilation/perfusion defect is identified in the right lung base (arrows second image) and corresponds to an abnormality seen on the chest X-ray (circle first image). In light of this triple match, this lung scan is associated with an intermediate probability for pulmonary embolism. (Also seen is central airway deposition of the radioaerosol.)
