Diagnosis: Cecal volvulus mimicking sigmoid volvulus
We see a largely dilated air-filled loop of bowel (asterisk) in the RUQ with a crease (arrow) giving it a coffee bean appearance. The crease is pointing towards the right upper quadrant which is more typical for a sigmoid volvulus. However, contrast enhanced CT (image to the left) confirms that the rotated structure is indeed the cecum – rotated around the cecal mesentery (white arrow). Black arrows indicate a (rectal) contrast filled non dilated descending colon.
