Diagnosis: Pneumatosis portal venous gas
Linear, radiating air lucencies are noted projecting over the liver (arrows, CT topogram at the top). These lucencies correlate within the intrahepatic portal venous tree (arrows, coronal reformatted CT in lung window, lower image). The air is transported via SMV secondary to pneumatosis, caused by the patient’s small bowel obstruction (see dilated bowel loops in the upper image).
