Diagnosis: Duodenal hematoma
Abdominal radiograph demonstrates an air filled stomach and dilated air-filled first portion of the duodenum (arrow, left image). Upper GI examination demonstrates a large mural filling defect involving the second portion of the duodenum which nearly completely obstructs the entire lumen (arrow, right upper image). Contrast enhanced CT of the abdomen demonstrates a large hemorrhage involving the duodenal wall, with some higher attenuation components, indicative of active bleed (arrow, right lower image).
